I am a family member of a resident and visit about once a week, usually in the evenings. What I have repeatedly witnessed at this assisted living facility is unacceptable and unsafe. Required daily wellness checks are not being completed. My relative requested a PRN medication and never received it. Trash is consistently left sitting in my relative’s apartment until I personally take it out, causing strong odors and unsanitary living conditions. Many caregivers appear disengaged. I frequently see staff on their phones and iPads instead of attending to residents. In the dining room in particular, this is a constant issue. There is a larger Caucasian woman who regularly sits on her phone, a tall, skinny African woman walking around on her phone, and another African woman serving meals while wearing an earpiece and talking to someone during service. This behavior is unprofessional and inappropriate in a care setting. My relative has been yelled at multiple times by dining staff. The larger Caucasian woman has raised her voice at my relative out of frustration when my relative did not understand certain dining rules. Additionally, the dining room supervisor yelled at my relative for wearing only socks in the dining room. What makes this especially concerning is that my relative has a documented medical condition that causes pain and swelling in their feet, making it medically unsafe and sometimes impossible for them to wear shoes. On those days, wearing socks is not a choice—it is a medical necessity. Instead of offering assistance, alternatives, or compassion, staff responded with yelling and embarrassment. This shows a complete lack of understanding of medical accommodations and resident dignity. Food safety is a major concern. On 01/12, my relative was served chicken that was undercooked and bleeding, which could have caused serious illness. Leadership behavior is also alarming. I have personally overheard the director and the maintenance man engaging in inappropriate sexual conversations near the elevators. Additionally, two residents witnessed the director pulling up her shirt and exposing her panty line while outside smoking, shortly before dropping my relative off in a dirty dining room that residents are expected to eat in. My relative has also reported that office staff, caregivers, and the maintenance worker drinking while on the job. These reports, combined with what I have personally witnessed, make it impossible to trust this facility with my loved one’s care. I have contacted corporate multiple times and left voicemails but received no call backs and seen no corrective action. Because of this, I am prepared to escalate these concerns to the state. I have kept detailed notes, dates, photographs, and documented conversations, including proof of PRN medication requests and documented staff behavior. I will remain anonymous due to fear of retaliation toward my relative, but these incidents are documented and ongoing. Jennifer in the front office has been professional and kind and is one of the few positives here. Carol, the nurse, also seems caring, though she appears to have limited authority. Unfortunately, they are the exception. This does not feel like a home—it feels like a place where residents are monitored but not properly cared for. This facility, especially the kitchen, needs serious oversight. A third-party food service provider such as Compass should be considered. I would never recommend this facility to anyone trusting them with the care, dignity, and safety of a loved one.

Summit Woods is a great place to live. The people in the office are very helpful and nice. Our activity director comes up with great activities for the residents. There are very friendly residents also. I’m very happy that I live here.

I think summit woods is out standing every one is nice and polite. Especially the E.D. she's very warm and caring very easy to talk to.summit woods is a very well run facility it should be a model of how things are done. And it starts with fantastic staff . there 2nd to none.

Great place to live. It like living in a small community. The staff here is just a great source of help.The apartments are nice and cozy. Lots to do here. Games, bingo, cards. Birthday party's. I do love it here!
