Ascension Living Resurrection Village Life Center

I've just returned home after a lengthy stay at this facility. The care I received was excellent. I felt supported, cared for, encouraged and safe. The nurses (Carmen, Josephine, Tonja & so many others) are my heroes. They were always pleasant and helpful. Their patients' care and comfort was always their priority. It was reassuring to know they were always just steps away. The CNAs (Francis, Jada, Alex, Precious, CeeCee, Nery & countless others) are top-notch. Whatever the CNA's are getting paid, it isn't enough. Talk about grace under pressure. I was constantly being checked on and they were always offering assistance. It felt good knowing someone is a "buzz" away 24/7. My PT team -Tina, Ben & Artie (or is it RT?) are the biggest reason I'm back on my feet and at home. Not only did they help me, but I watched them and their co-workers (Luis, Natalia, Kelly & Sonia - hope I got the spellings right) work with so many patients, who required different levels of therapy. They work miracles. The Wound Care Team - Dr V & Co. are incredible. I received care from them 7 days a week for months. They were attentive and meticulous. I was saddened to learn that my last day at the facility was also their last day, because the Wound Care Team was disbanded. I'm so very disappointed and think this is a mistake. Housekeeping - Erica & Ralph. Not only did they keep things immaculate, they would always be there to give a kind word or a smile. They were beyond friendly and helpful. Also, the person that took care of my laundry, I think her name was Dani (if I'm wrong, I apologize) was great. Stephanie - Nurse Practitioner. She has the best bedside manner. Any problems or concerns, I knew she would be there (even on one good foot, at times) to offer the right advice and care. The office team - Hannah, Monica, Michelle & Kate were always on top of everything. From booking transportation for medical appts to answering billing questions to making sure I went home with proper medical equipment. Their attention to detail was flawless. Honorable mention - the ladies who made sure I always had an updated calendar or fun puzzles to keep me busy. They always brightened my day when they'd show up at my door. Let's be honest, we've all heard horror stories re: nursing/rehab stories, so I was nervous when I learned I would need to reside in a facility for an unknown period of time. I'm so grateful that at a time I was incapable of taking care of myself, I was able to receive care at a top-grade facility.

My mom Nina was admitted to the life center for the past three weeks. As a senior citizen she received excellent care from RN practitioners like Stephanie Schnell and sound advice from her social worker Michele. As for the facility it was in fair condition as a section was being renovated, there were no issues or excessive noise from crews. We usually think food isn’t the greatest but my mom appreciated the meals and the attentiveness of the assistant teams. Physical and occupational therapy teams were also understanding and helpful with getting mom through her sessions. When I sat in on the staffs meeting to discuss mom’s status, they were true professionals and answered my questions; to include recalibrating moms meds based on side affects. One constructive observation, some of the older seniors with memory issues were parked in wheel chairs throughout the hallways asking for help especially at the nurse station. It made it challenging at times to ask questions. The staff still managed around them but those poor people really needed assistance first.

I just wanted to give a big shoutout to Monica Hernandez in Social Services she has been absolutely wonderful to work with. Whether it's helping residents with their day-to-day needs or navigating the discharge planning process, Monica always shows up and gets it done with a smile. I've also had the chance to work with Michele Yeager from Social Work Consultation Group recently, and honestly, the two of them together are a great team. Even when things get busy and the environment is moving fast, they make the whole discharge process feel smooth and easy and you can tell they genuinely care about their patients. And even with the transition of Ascension Resurrection Hospital to Prime Health, the care inside this building hasn't skipped a beat. The nurses here have continued to go above and beyond for their patients, and that says a lot about the people working here.

I had the pleasure of visiting this community and was very impressed with the services offered, the staff and the building itself. All the rooms are private, which is so important and rare for folks when they are rehabbing. The admission Manager, James, is knowledgeable and very willing to get your questions answered. I would not hesitate having a loved one come here for rehabilitation or long term skilled care.
