I just wanted to give a big shoutout to Monica Hernandez in Social Services she has been absolutely wonderful to work with. Whether it's helping residents with their day-to-day needs or navigating the discharge planning process, Monica always shows up and gets it done with a smile. I've also had the chance to work with Michele Yeager from Social Work Consultation Group recently, and honestly, the two of them together are a great team. Even when things get busy and the environment is moving fast, they make the whole discharge process feel smooth and easy and you can tell they genuinely care about their patients. And even with the transition of Ascension Resurrection Hospital to Prime Health, the care inside this building hasn't skipped a beat. The nurses here have continued to go above and beyond for their patients, and that says a lot about the people working here.

Stay away. My mom was there 7 days, rehab, and after 3 days I attempted to get her out. Left in bed, not dressed, not showered, filthy room, forgot to give her her dentures to eat, given medication not suppose to have and we were there every day and it didnt matter. Could not get ahold of anyone there. Called social services a third time and was told if I took her out ama they would charge for the days there. Left 3 messages for the Executive Director, no return calls. Finally threatened to call Senior Protective Services and on day 5 the doctor, nurse manager and social worker showed up in her room. I had never seen or talked to any of them before that even though I asked. More excuses, more incompetence, finally day 7 got the PA, extremely helpful, to discharge her. Many of the staff are from an agency, different people daily, stay away. Today is 01/10/2026!!!! Left this week's ago, automated reply to call, which I did. Left a message to please call me back, NOTHING! NOT A CALL! Not from this number, nor the Executive director who I left numerous messages for. Not a peep from anyone. They just dont care and are incompetent. Again stay away from this place.

I had the pleasure of visiting this community and was very impressed with the services offered, the staff and the building itself. All the rooms are private, which is so important and rare for folks when they are rehabbing. The admission Manager, James, is knowledgeable and very willing to get your questions answered. I would not hesitate having a loved one come here for rehabilitation or long term skilled care.

Parking is a nightmare since they painted the street curbs on Oconto yellow. The angry nuns will put a tersely worded sticker on your window if you park in the street. The parking lot itself is inadequate for the number of visitors. So you will have to go park in near Amazing Grace building or further down the street. Nobody is happy about the recent parking mess
