This isn't my fist time visiting Smith's Mill. Around 3.5 years ago my grandmother was in the memory care unit for a short stay after a fall. The facility and staff in the mc unit were great back to my grandmother even though she was nonverbal due to Alzheimer's. My interactions and time at the facility were brief but they stood out because of those positive interactions. My 2nd mom who I've been taking care for the last several years is currently a resident at Smith's Mill for rehab. Unfortunately her dementia has progressed so I wasn't sure what to expect from her once she was transferred from the hospital. She's slowly perking up except for when Ashley Everson stops by her room each morning to check on her. She's much more talkative with Ashley and more like herself. That alone was enough for me to leave a review. As former fire/ems, I've seen all kinds of facilities and staff so I came in to sign paperwork and ready to ask a hundred questions. I left without asking any of them because Ashley covered everything I was going to ask and went over things I hadn't thought to ask or even thought about. It's rare to find a facility that is as nice as they appear on the outside. It's even more rare to meet someone like Ashley that genuinely cares about not just the residents but making sure the family members are informed and kept in the loop. When visiting my mom, I saw Ashley pass by the room and stopped her to ask the various units and then asked about meds because my mom said she hadn't been given any pain meds. I took that with a grain of salt because dementia. Ashley could have said she would send the nurse in to talk to me about meds but instead, she talked to the nurse and let me know when the last dose was given and how long before my mom could receive another dose. In that same visit, I was greeted by people left and right while walking to my mom's room. I brought her dinner for her birthday and she was just finishing it when Myshawn came in with dinner and wished my mom a happy birthday. She was slightly confused so he showed her the meal order with her info on it and the happy birthday at the top. Asked how her birthday was and if she enjoyed her dinner then said he had a piece of cake for her. She was all smiles during that interaction with him. Those moments where she's smiling ear to ear while in rehab are fleeting so it means a lot when that happens. I've told a number of friends that have aging family members about my experience over the years any time my mom goes to an SNF so they have options when needed or when they start the process of moving a family member who needs more care. I've since told all but a few of them about Smith's Mill and if they decide to check it out, ask for Ashley because she is not only a font of knowledge when it comes to elder care but she is genuine when it comes to caring about the residents. That and the food has looked delicious every time I've been to visit during meal time. Honestly a little envious of my mom on that front.

Smith Mill was the best facility my Dad stayed at in his final phase of life. The nurses were caring and admin helped us through all the care and financials. No facility will be perfect - but if you show up as part of the team , Smith Mill will be a key part of that care team to ensure good care for your loved one through a rehab or final stages of life.

If you are looking for a facility that will care for your loved one please do NOT move them to this facility!!! I really feel bad for a FEW of the nursing staff who do care and really tried to do what was needed! We had wayyyy too many “accidents”, “falls” to even keep track of. I’m sure if I could still access our loved ones records I could list ALL of them. But, I couldn’t get into them the last day she was a resident (she went straight to the ER and we didn’t move her back to Smith Mill). The hospital was very gracious to keep her until we could find a new facility, because they didnt want her to go back to Smith Mill!!! The last straw was my daughter and I walking into her room (her door shut) for a visit and she was PINNED between her bed and the wall UPSIDE DOWN!!! I screamed and the nurse walked in and casually said, “we were just in here 20 min ago, that happened quick” To say I was furious is an understatement!!!!! She then spent several days in the hospital from this incident. Many others prior to this incident!!!! Then you have to deal with Seth, admin. He should NOT be in charge of this type of a facility!!!!! Never! I mean NEVER a returned call when he says he will “look in to something”. Or even a email when told he will follow up through email!!!!!! And then you have the billing department! If you currently pay them money I sure hope you triple check what they are billing you!!!!! Our mom passed soon after we moved her out of Smith Mill. After MANY DISCREPANCIES in her billing I went INPERSON to see Seth! He gave me a final bill and I paid in person to him!!!! I made him write “Paid in full” on my receipt with his signature. That was in Nov or December. I am STILL receiving bills for services back in April of 2025!!! I did call home office and spoke with someone who gave me a number to call and follow the prompts. I am sorry! But I am NOT calling another number to leave ANOTHER message for them NOT to return my call!!! As you can tell the frustration we have had with Smith Mill!!!! Please do not put ANYONE you love or don’t love here!

Smith's Mill Health Center has a wonderful Therapy Department. It is because of PT and OT that I am able to walk today. When I arrived at Smith's Mill I was not only bed-bound and couldn't move my left leg, I was also depressed and felt no hope for my situation. Therapy didn't give up on me. They cared and helped me work through my limitations and fears. I also want to say that the Life Enrichment(Activities) Department have some of the most hardworking staff who plan wonderful activities. Casino night was amazing. So many activities were planned to do that I was never bored. The Nutrition (food service) Department went above and beyond to make sure that my dietary needs were met as well as preparing delicious meals. In ending, I would like to thank all of the staff at Smith's Mill.
