Licensed nurses and certified caregivers are available 24 hours a day to help with bathing, dressing, eating, mobility, bathroom use and medications.

TWO STYLES OF ASSISTED LIVING
Choose from our two styles of assisted living communities:

Traditional. Residents live in studio or one-bedroom suites, including private bathrooms and kitchenettes. Traditional assisted living is available at Pleasantview Care Center.
Medical model. For residents who need extra medical attention, the medical model is a comfortable mix of independence and hands-on care. Residents live in spacious single-room suites with private bathrooms. Medical model assisted living is available at Parkside Villa and Pleasant Lake Villa.

    My mother was very pleased with the nursing staff and care. Sara RN was extremely skilled in identifying and notification to the doctor for IV care that was administered in-house. My mother received excellent therapy care and was strong enough to return to her assisted living. I cannot say enough wonderful things about the cleanliness and caregivers. Their heart is in the right place and were always kind. Thank you for a positive experience! Please share with your staff.

    Was there for four years, overall the staff was excellent,however there were too many aides that didn't care about the residents, most nurses were great like Debbie, Pam, Jessica, and Mary. Miss Jackie is probably the best employee of all along with Shauntina and any residents that were under their care were truly blessed. Meals were terrible and social services were inexperienced and not quite great at dealing with people, however Kristy tried her best. ADON Jessica was on top of things and her authority was respected along with a willingness to help however she could, executive director Sharon was overworked and did the best she could with what she had to work with. Activities were fun, however repetitive and the housekeeping was top notch.

    Incontinence supplies in trash cans, water damage in rooms being band aided, unsanitary, walls were disgusting, carpeting has seen way better days… call light across the room where residents can’t reach, check on resident every 2 hours…? not happening… there is no excuse for someone to sit in a chair for 8 hours straight because they can’t reach the call light and they were only checked on because of tray line and told well we can’t do anything while we are passing trays, but even when trays are picked up they are still sitting in chair waiting for them “to come back”… overflowing laundry barrels in the hallways… aides sitting out in hallways/ day areas on their phones and eating… dishwasher broken for weeks, residents receiving cold food no offer to open or cut anything or to reheat, food carts are being left wide open as aides are passing trays, no water or juice or anything else to hydrate being offered unless family visits or nurse is giving water with meds… also if patients have a history of aspiration pneumonia or have trouble swallowing should they not be up in a chair or at least boosted up in the bed enough so they don’t choke or aspirate again?????? Last time I checked when the food is above my head and I’m lying flat in the bed I cannot even see the food let alone eat it. There is some staff that are pretty good on some days but their shifts only last so long. Staff sitting on bench at front door smoking. All the things that ODH doesn’t get to see.

    Worst facility l've ever encountered. No matter what time you call they NEVER answer the phone. They need to be investigated due to lack of care and responsiveness. My Uncle, who is low maintenance, would press the call button to no avail. I went to what was supposed to be a care meeting (it ended up being an "I DON'T CARE MEETING "). No one came except an office coordinator..... no nurse, no therapist, apparently they were too busy for a scheduled meeting. Laundry never came back, don't know what happened to his visibly marked clothing. They were supposed to administer a flu shot, he was there over 2 weeks and never received the shot. No matter who you talk to the right hand doesn't know what the left hand is doing. Beyond frustrating. The ineptitude is stunning.
