Harmony Trace is a nonprofit assisted living and memory care community in Hilliard, Ohio, offering 24/7 support, studio and one-bedroom residences, meals, medication support, personal care, life enrichment, and therapy access.
Assisted living at Harmony Trace is designed for older adults who want independence but need reliable help with daily routines. Residents can receive support with bathing, dressing, grooming, medication management, meals, mobility, housekeeping, laundry and wellness activities. Our goal is to help each resident stay as independent as possible while giving families peace of mind that support is available 24/7.
Harmony Trace Memory Care supports older adults living with Alzheimer’s disease, dementia, or other memory changes. Harmony Trace is the first Teepa Snow Positive Approach to Care designated community in Ohio. Residents receive 24/7 support from a trained team, personalized daily routines, medication management, assistance with bathing, dressing and incontinence care, memory cues, mealtime support, and activities designed around each resident’s abilities and interests.
Families can feel confident knowing their loved one is supported throughout the day and night—not just with care tasks, but with dignity, patience, nutrition, engagement, and familiar routines, all within a community that has been a trusted provider in central Ohio for over 30 years.

My wife has resided at Harmony Trace for 20 months. The staff has been terrific for the entire time. I visit her every evening, eat dinner in the dining room, take her out for rides in the area, and visit with her and the other residents. The general attitude of the facility is very upbeat and positive. The aides and nursing staff are always available to give assistance. Definitely a five star facility.

Harmony Trace is outstanding! My mother received excellent, loving care during her 6 years at NCR. I recommend the memory care unit where residents live in a vibrant, stimulating environment and are treated like family by each staff member. If your loved one is in need of memory care - this is the place to be. I can only give them 5-stars but if I could, I would give them 5 gazillion stars - they are that good!
