Overview
Sophie's Manor is a medium sized CBRF - with a maximum occupancy of 20 residents. Sophie's is located within the small town of Centuria, WI - Polk County. It has been family owned and operated for the last 18 years. We have highly trained staff, excellent homemade food, and a fantastic management team. Our RN Administrator is on site full time along with a full time Assistant Administrator. We also have an activity department, maintenance, and housekeeping.

Our pricing is very competitive for our area. There is a point system that will reflect the individual needs of the resident. The range of pricing is $3900.00-5600.00 per month maximum. We accept family care as a funding source and are contracted with 2 different MCOs. We have no hidden fees for move in or move out or completing an assessment.
