Extremely frustrating experience. Gave two stars for the PT and OT staff which were excellent. But my loved one only received one bath in almost 4 week stay. And I had to ask why it wasn't being done. Staff seemed resigned that information and communication would spotty and confusing at best. Mom waited almost an hour on the toilet one day when no one came back to help her back to her wheelchair. I could go on and on. Would love to talk with administration if they care to reach out.

Overall I found Forest Spring to be a good to very good rehab facility. I was a resident for two periods of time in 2025.
