CountryHouse in Elkhorn opened in January 2021. Our gorgeous community is designed purposefully for those with dementia. At capacity, we are home to 40 residents who are cared for by a team specifically trained to help those who live with the challenges brought forth by memory loss. A sister community, in nearby Millard, is just six miles away.

Pricing

Starting Price Starting Rates: Base rates for a companion room start at $5995. Base rates for a private room start at $7295. Respite care rates start at $275 per 24-hour period. Day stay visits are $125 a day.

Base rates at CountryHouse include your choice of apartment and all residential services including daily meals and snacks, housekeeping, activities, transportation, utilities, life enriching activities and much more. Additional health care is available for services such as medication delivery, bathing or dressing assistance and more. Prior to moving in, individuals are assessed by by our wellness director to determine what kind of health care services will be needed to live successfully at CountryHouse. Each health care service is assigned a point value that is based on the number of minutes it takes to perform the service as well as the level of expertise of the staff person providing the service. We offer four care levels. Level one of care starts at $750 monthly with costs increasing as levels get higher Care beyond level four is available and charged based on minutes and points. Please ask our team for more details. Other Cost Considerations: A community fee of $2,500 secures the room of your choice for up to 15 days. This is due prior to move-in. It is not refundable after 30 days of residency. A $500 deposit adds you to the waiting list for our community and is applied to your first month’s rent. This deposit is fully refundable if a move does not occur. Families are responsible for services not normally provided by CountryHouse, such as personal telephone or salon services. Additional fees may be applicable for enhanced care, personal attendants, premium rooms or pets. Yearly cost-of-living increases do occur. Memory Care

Memory care communities offer specialized care and support for individuals dealing with memory-related conditions, such as Alzheimer’s or dementia. These facilities are equipped with trained staff and offer engaging activities designed to promote well-being while ensuring a safe living environment.

Respite Care

Respite care is short-term or temporary care. Respite care can be provided in adult day centers, in private homes via home health agencies and in senior living communities, most often assisted living or skilled nursing facilities.

    Highly recommend Country House. Excellent staff of caring professionals who provided such good care for our often difficult father. Clean, well lit, open facility and fantastic meals. Great communication. Many activities in which the residents participate together. Can’t say enough about how this staff took Dad in and really raised his spirits for most of his stay. Thank you!

    Excellent care. Caregivers are always attentive, compassionate and thoughtful. Food is consistently superb. Residents get to participate in many fun and innovative activities. Facility is new and is an inviting and warm environment.

    Felt the need to move my 94 y/o mother from this facility after less than a year. Major reason was she had fallen sometime during the night and we received no notification. Only discovered after a visit later in the day and noticed a knot on her head. After the fall she had been put in bed which is NOT recommended after a head injury. Initially, the Nurse denied she had fallen but when one of the caretakers verified it the blame was placed on the night shift therefore it wasn't her problem. Discussed the issue with the facility manager who promised to get some answers and get back to me. Two days later she made contact requesting a meeting at which we were expecting to get some info regarding the incident. However, that wasn't the case. The meeting was to inform us of the $1,000 + rate increase they were going to add due to "extra needs". There were issues prior to this incident, ie: unclean toilet, dusty furniture, Rx mix-up, but the lack of notification, allowing her to go to bed with a head injury, and no promised follow up made it evident a change of venue was needed which we did February 1st.
