CountryHouse in Elkhorn opened in January 2021. Our gorgeous community is designed purposefully for those with dementia. At capacity, we are home to 40 residents who are cared for by a team specifically trained to help those who live with the challenges brought forth by memory loss. A sister community, in nearby Millard, is just six miles away.

Pricing
Starting Price Starting Rates: Base rates for a companion room start at $5995. Base rates for a private room start at $7295. Respite care rates start at $275 per 24-hour period. Day stay visits are $125 a day.
Base rates at CountryHouse include your choice of apartment and all residential services including daily meals and snacks, housekeeping, activities, transportation, utilities, life enriching activities and much more. Additional health care is available for services such as medication delivery, bathing or dressing assistance and more. Prior to moving in, individuals are assessed by by our wellness director to determine what kind of health care services will be needed to live successfully at CountryHouse. Each health care service is assigned a point value that is based on the number of minutes it takes to perform the service as well as the level of expertise of the staff person providing the service. We offer four care levels. Level one of care starts at $750 monthly with costs increasing as levels get higher Care beyond level four is available and charged based on minutes and points. Please ask our team for more details. Other Cost Considerations: A community fee of $2,500 secures the room of your choice for up to 15 days. This is due prior to move-in. It is not refundable after 30 days of residency. A $500 deposit adds you to the waiting list for our community and is applied to your first month’s rent. This deposit is fully refundable if a move does not occur. Families are responsible for services not normally provided by CountryHouse, such as personal telephone or salon services. Additional fees may be applicable for enhanced care, personal attendants, premium rooms or pets. Yearly cost-of-living increases do occur. Memory Care
Memory care communities offer specialized care and support for individuals dealing with memory-related conditions, such as Alzheimer’s or dementia. These facilities are equipped with trained staff and offer engaging activities designed to promote well-being while ensuring a safe living environment.
Respite Care
Respite care is short-term or temporary care. Respite care can be provided in adult day centers, in private homes via home health agencies and in senior living communities, most often assisted living or skilled nursing facilities.

I was really disappointed with my loved ones experience here. I had recently had them in a previous senior living home and was promised it would be better here.. it wasn’t. Firstly I should have seen the first red flag when they called me numerous times and sent me letters, emails, and texts . I ignored the red flags and shouldn’t have. With that being said the cost is extremely high for the level of engagement and services provided. There seems to be very little meaningful interaction from staff or if there is they are always on their phones or not engaging with the patients , which is concerning given what families are paying. It felt like my dad was just a price tag. And the cost kept going up. The levels they charge are insane ! Additionally, the management team is rarely present or accessible, making it difficult to address concerns or feel confident in the leadership. There also appears to be a high staff turnover rate, which impacts consistency of care and the trust . For the price, I expected much more in terms of involvement, stability, and support. I would encourage others to carefully consider these factors when making a decision, go elsewhere .

CountryHouse Memory Care in Elkhorn took excellent care of my mother during the last month of her life in October and when she was on hospice. I could not have asked for better. Their unit staff on every shift, the health aides, CNA’s, and med aides are great, and consistently went above and beyond not only for my mother, but for me also and they didn’t have to do that. They truly care about their residents. I saw it every day, all day long, the little extra sweet things they would do to make a resident feel safe and like they mattered greatly to them. I’m so appreciative to the CountryHouse owners, Agemark Senior Living, and their CEO, Marty Hug, for always being so wonderfully hands-on, and putting every single resident’s best care first at every one of their facilities. Also making every family member know that they are so important to every resident’s well-being. Marty is the best in the business. My mother was at a different Agemark facility in Omaha for three years from December 2019 to September 2022 - and there she also received the same outstanding care and sincere compassion from staff member. Especially during Covid. My family and I are grateful beyond words. Thank you.

Highly recommend Country House. Excellent staff of caring professionals who provided such good care for our often difficult father. Clean, well lit, open facility and fantastic meals. Great communication. Many activities in which the residents participate together. Can’t say enough about how this staff took Dad in and really raised his spirits for most of his stay. Thank you!

Excellent care. Caregivers are always attentive, compassionate and thoughtful. Food is consistently superb. Residents get to participate in many fun and innovative activities. Facility is new and is an inviting and warm environment.
