Overview
Just because your loved one is living with Alzheimer's or dementia doesn't mean they can't enjoy a fulfilling life. At Brookdale Nacogdoches, our memory care program is rooted in a person-centered approach that helps strengthen skills and personal sense of identity—all while having fun. You'll find our community nestled in warm, sunny San Antonio, Texas, with convenient access to nearby shopping and restaurants.
At Brookdale Nacogdoches, we gently guide our residents through a structured daily path of engagement composed of a minimum of six activities designed to strengthen cognitive, physical, and social skills. Some of these activities may include clustered group discussions, creative art classes, and walking groups.

And to help ensure that your loved one feels right at home, you'll be able to choose between multiple apartment floor plan options before move-in day. No matter which floor plan you choose, your loved one's new home will have a private bathroom, spacious closets, and large windows to let in natural light. We're also a pet-friendly community, so all four-legged best friends are more than welcome—because we know it just wouldn't feel like "home sweet home" without them.
When it comes to your loved one's care program, our specialty-trained care staff is ready to help with grooming, dressing, medication management, getting around campus, dining assistance, and more. Throughout the day, we're also here to provide gentle reminders and guidance when needed.
And, we have care staff on campus 24 hours a day, seven days a week, to be alerted to any emergencies.

Brookdale Nacogdoches has been a blessing for my family. My mom has been there for almost 2 months and the love and care she is shown every day touches my heart. They are kind, compassionate, and care for her like she was their own. We have cameras in her room and it warms my heart to see the interaction between my mom and her caregivers. She is fed nutritious meals and snacks and her room is always clean. It is the interaction with the residents that all of the staff of Brookdale have that truly makes it shine.

My uncle was a resident at Brookdale Nacogdoches for a many years and was always in good hands with their staff. As a veteran, he was ALWAYS treated with dignity and respect and even had his photo framed on the Veteran wall, in honor of his service. As his health began to decline, the staff made the necessary adjustments to provide him with the assistance that he needed when it mattered most. Everyone was wonderful in his final moments and I’m so grateful for their care.

My mom lived here for about 7 months. The care was subpar to put it mildly. She was often not clean. Staff would lie about changing her or brushing her teeth. We would often find her clothes on other residents, even though we did all her laundry. Staff didn’t know how to maintain her oxygen equipment. They charge for personal services not being performed. Felt bad for residents who didn’t have family who regularly visited and could observe what was going on. Management would always apologize and state she would train staff when they messed up. Residents can exit all exterior doors easily. Hallways smelled like urine. I could go on and on…

The staff is amazing and how they care for their residents and they keep them interactive with activities to help with their sleep pattern and they offer choices for meals as many other positive things about this community.
