Located in Avon Lake, Sprenger Health Care Towne Center is a state-of-the-art facility featuring a contemporary atmosphere. This active community offers daily life enrichment activities and modern amenities including a pool, putting green, private dining rooms, and on-site salon.
Our purpose at Towne Center is to offer exceptional care, at various levels, to the seniors in our community. Uniquely designed as a continuing care community that includes an Independent Living Facility, Assisted Living Facility, Skilled Nursing, Long-Term Care, Memory Care Assisted Living and a Memory Care Unit.
Traditional Assisted Living offers a wide array of services and amenities including prepared meals, socialization life enrichment activities, medication management, laundry service, outpatient therapy, and on-site nurse practitioner visits.
Parkway Memory care was designed specifically with safety and continuity of care in mind, optimizing quality of life and encourage residents to maintain a healthy level of activity. Some of the services and amenities include special trained staff, medication administration, activities directed to residents with memory loss, secured unit, daily living care, three meals per day, and on-site nurse practitioners.

We transferred our mother here 3 months ago because she was being neglected at her last facility. I hesitated to put her through the trauma of a move due to her mental condition but knew I couldn’t leave her where she was either. I thought all these places are the same and I’m just going to uproot her to the same. I couldn’t have been more wrong. This facility is like night and day from where she came from. There is always plenty of very helpful staff who are present, attentive, and involved (unlike the last place who had maybe 1 aide who sat with her phone in her face the entire time she was working). The place is always clean with no smells. My mother is clean with no smells. If I ask for something it gets done, and it gets done right away. I had a care plan review on day 1 and again after 3 months (it had been a year and a half since I had one at the last place). My mother is happy and has adjusted very well to her new surroundings. And I promise you … I am NOT staff being paid to write a 5 star review. This is well deserved. I am very happy with my mother’s care here. When you come from and see bad care, you know what good care looks like.

This is a BEAUTIFUL FACILITY but their staff and management on BOTH SIDES needs to be terminated...LIKE YESTERDAY! I think all these 5-star reviews are from employees that work there and they aint worth the money they get paid. Like some have said, these 5 star reviews are fake! Staff, especially the aides, are LAZY!!! The scheduler doesnt do her job well on the longterm side, always severely underataffed. When she has to work the floor, she doesnt help either. Managment sit in their office all day and does ABSOLUTELY NOTHING!!! They have no clue what goes on bc they wont come out of their office. They think they are too good. No team work! The place is not clean. Residents food is served cold and they dont put lids on top when serving the food. This place doesnt deserve good staff. That is why they are always leaving and the residents are left with the mediocre workers. Give me a hole in the wall place with solid staff any day of the week! State always get called bc of the neglect that goes on here! If you hate your family, send them here!!!

The out patient therapy was great. OT Angela was very kind and taught me a lot about how I can continue with my home exercises.

Been here for the past 2 months and I love it here!! The staff is friendly and very welcoming, always willing to help out and get the job done. The DON and ADON have a known presence around the building and are always willing to step in and help if needed. The nurses are super helpful and love lending a hand with care. The environment at town center has a family feeling and it feels great being apart of this amazing community.
