This past winter we had to make a hard decision that we all dread. Putting a parent into a care facility. After touring Autumn Glen we decided this would be the best chose. Our Mom was not happy so she was a handful, however we had no choice. It took a couple of weeks for her to get over being upset but soon she was thriving. She loves to tell us all about her favorite staff members. (Which seem to be all of them) She was not a very social person, but now she seems to talk to everyone. It makes all of us so thankful that we found this facility. Highly recommend! If you are looking for a place where your loved one will feel like they are part of a family. My siblings all agree! Hallelujah!!

Staff abused my mom. An over night nurse, left my mom sitting in her wheel chair overnight and would not provide assistance to help her to her bed, one week out from having rods and pins put into her hips/femur. I was told by head of nursing that they were able to provide the care that my mom needed. Not staffed properly for services that were deemed necessary by head of nursing. Their excuse was, “the on-call person was sleeping and did not hear her phone ring”. Thus, it put the facility at an illegal amount of overnight staffing needing as required by the State of MN. We were told they would need to look into their procedures, but it happened on numerous occasions. Missing medications and refusal of nursing staff to administer medications. The staff that are actually administering the medication did not follow the directions given by head of nursing, even after multiple attempts to communicating the instructions to her staff. Missing medication and pills found were wrapped in a napkin and placed in a nurse’s pocket. When I asked about this witnessed event, they just said they would have to look into it, but no follow up provided and upon leaving, there was medication ordered, paid for but not accounted for. The director, Penny was made aware of all of the illegal and abusive situations. When I brought up that they were mandated reporters and it should be brought to the attention of the proper agencies, she said they do not do that but instead do an internal review. They agreed that mistakes were made, illegal staffing was provided, abused/neglect occurred. Their response, “Please let us try to do better,” Please do not leave your loved one in their care. A beautiful facility does not equate to quality care.

Beautiful facility with spacious rooms. Jessica was fantastic in assisting us getting both my Dad and my Aunt in. The nursing staff is wonderful as well. The food and the kitchen staff are amazing. If you are looking for a real community feeling for a senior person look no further.

If you are looking for a memory care facility for a loved one, you and your loved one can do better than Autumn Glen. My father, who has Alzheimer's, was a memory care resident from May 2024 through November 2025. I was informed on two separate occasions (one occurring late in the evening) that he had somehow gotten out of their "secure" memory care wing and was found wandering the building. There was nothing preventing my father from simply walking out of Autumn Glen's front door. When asked how this happened, no one on staff could provide a clear answer. I was just told "maybe we need to get that door checked out." We lost confidence that Autumn Glen could provide a safe residence for our father and moved him to a different facility.
